1. background
The Fred Hollows Foundation (FHF) is a secular non-profit public health organization based in Australia, founded in 1992 by eminent eye surgeon Professor Fred Hollows. FHF focuses on strengthening eye health systems and the treatment and prevention of avoidable blindness. FHF operates in 25 countries across Australia, The Pacific, South and South East Asia, and Africa.
Since 2008, FHF has operated in Lao PDR in partnership with Provincial Health Department, Provincial hospital and the Eye Division under the supervision of Department of Health Care and Rehabilitating and The National Ophthalmology Center, and University of Health Sciences, Women Unions, Gender Development Association, and development partners and other NGOs to eliminate avoidable blindness in Lao PDR.
2. Purpose
To ensure avoidable blindness in Laos PDR is addressed in both short and long-term, with the funding support from Australian Government through Australian NGO Cooperation Program (ANCP), The University of Health Sciences and The Fred Hollows Foundation in Laos is inviting legal registered entities to submit tenders to provide ophthalmic equipment specified below:

Important Conditions:
– Bidders should be legal and registered companies or businesses. Patent and other registered documents should be included.
- Place of delivery, installment and usage training: University of Health Sciences (President Office, next to the French Embassy to Laos), Vientiane Capital of Laos.
- Source of capital: All equipment shall be paid by FHF
- Bidding price and payment:
- According to the provisions of the Law on Value-Added Tax (Revised Version) No. 52/NA, dated 23 July 2014 by the National Assembly of Lao PDR and the Instruction on Value Added Tax No. 007/MoF dated 11 January 2017 by the Lao PDR Ministry of Finance, in regard to reception and use of non-refundable aid of international NGOs, all goods and services purchased for projects including domestic purchase or importing shall be exempted from VAT and import tax. Therefore, prices offered in bidding document should exclude VAT and import tax; and include the costs of transportation, installment, technical training and usage guide for direct users at the places specified and for the equipment listed in section 2 above. VAT invoice must be provided before the payment is proceeded.
- Discount can be given directly in the bidding price (state clearly that the price is already discounted). Discount given in the form of commission or letter of discount if available please attach them in the sealed bidding document pouch. Every letter of discount and commission submitted after bids opening are invalid.
- Time and mode of payment: Payment shall be made in accordance to terms and conditions of the contract signed by the parties involved.
- Specific requirements for quotation of the requested equipments:
- Products to be quoted must meet technical specifications and other requirements of the equipment listed in section 2 above.
- Provide enough information of each product, including: name of product, model, technical specifications and configuration satisfactory to the request, functions and uses, quantity, unit price in USD, name of manufacturer, manufacturing year, product code, accessories (if available), warranty policy in accordance to manufacturer’s standards, estimated time to provide warranty services when problem occurs (estimate number of working days from the date receiving the request for warranty).
- Submit the price quotation in both hard copy with authorized signatory and the company stamp and soft copy (to be copied to a CD or USB) for easy data handling later.
- Provide catalogue of the product quoted.
- State clearly delivery time from the date of contract signing for each specific item in the list.
- State clearly valid time of the quotation, but not less than 30 days.
- Your capacity profile, if available, will be an advantage for bidding selection.
- The envelop of the dossier must be sealed with stamp.
- Method of submission: directly at FHF office or by post.
- Delivery requirement:
- Supplier is to send detailed delivery plan/schedule to FHF Office for review and agreement prior to the delivery.
- It is required that FHF staff should supervise the delivery and installment of goods, and training of usage to recipients. Equipment should be delivered together at a time in order to easily provide trainings on technique and usage of the equipments for direct users and conduct handover ceremony.
- FHF and recipients reserve the right to refuse inspecting the goods if the supplier does not obey strictly the requirements on delivery time and/or goods specifications as agreed in the contract signed by the parties involved.
