Request for Quotation for Health and Personal Accident Insurance

1.Background

The Fred Hollows Foundation (FHF) is a secular non-profit public health organization based in Australia, founded in 1992 by eminent eye surgeon Professor Fred Hollows. FHF focuses on strengthening eye health systems and the treatment and prevention of avoidable blindness. FHF operates in 25 countries across Australia, The Pacific, South and South East Asia, and Africa.

Since 2008, FHF has operated in Lao PDR in partnership with Provincial Health Department, Provincial hospital and the Eye Division under the supervision of Department of Health Care and Rehabilitating and The National Ophthalmology Center, and University of Health Sciences, Women Unions, Gender Development Association, and development partners and other NGOs to eliminate avoidable blindness in Lao PDR.

2. Purpose

To ensure that FHF staff received best benefit and quality of the service about the policy of indemnity, The Fred Hollows Foundation intends to enter a contract to new Health and Personal Accident Insurance for FHF staff in 2022. Please see some detail on below.

Important Conditions:

• Submitters should be legal and registered companies or businesses. Patent and other registered documents should be included.

3. Policies, quote price and payment:

• The insurance companies or servicer must provide the policies of indemnity, the benefits of the insured, what are the cases that can be compensated, what cannot, etc… and attach in quotation document (full policies).
• Special service or additional service can be given directly in the quote or add in the policies and state clearly sentences.
• Discount given in the form of commission or letter of discount if available please attach them in the quotation document pouch. Every letter of discount and commission submitted after quotations opening are invalid.
• VAT invoice must be provided before the payment is proceeded.
• Payment shall be made in accordance to terms and conditions of the contract signed by the parties involved.

If you are interested, please submit your quotation in hard copy to the office of FHF at 6nd Floor, Unit 601, Royal Square Office, NongDuang-Nua Street, Nongduang-Nua Village, Sikhottabong District, Vientiane Capital, Lao PDR. However, due the situation of Covid-19, you can submit in soft copy via pdorasing@hollows.org by Monday 01 November 2021.
